If you are unable to keep an appointment, we ask that you kindly provide us with at least 48 hours notice. We ask for this advance notice so that we can offer this appointment to another patient. A $50 charge will be applied for missed appointments or repeated short notice cancellations. We confirm all appointments so please assist us by returning our call if we are unable to reach you directly. We try our best to be on time for your appointments and ask that you do the same in consideration of the patients booked after you.
We require payment on the day of service, unless other arrangements have been made in advance. Please be aware that treatment estimates are always approximate and the amount may change due to unexpected circumstances such as lab costs or unforeseen changes to your treatment plan. We will do everything we can to help you afford the treatment you need and want. For patients who require major work, we may be able to help with an appropriate payment schedule. Forms of payment accepted by the office are cash, debit, VISA or Master Card.
We will assist you by completing and submitting your insurance claims and in most cases accept assignment of benefits, but as the receiver of treatment you are responsible for payment. We expect prompt payment of all rejected claims. If you do not provide us with the required insurance policy details to enable us to submit to your insurance plan, you will be required to pay your balance in full.
Your dental insurance is a confidential policy between you and your insurance company. Due to privacy laws we are unable to access information from your insurance company, so please provide us with all the details of your insurance plan. You also need to be aware of the limitations of your plan to prevent unpleasant surprises. We do our best to monitor plan limits, etc. but ultimately this is your responsibility. Please advise us if there are any changes to your insurance coverage.
We can obtain limited information by submitting pre-treatment authorizations to your plan but we have no way of knowing many of the clauses and limitations of your policy until a claim is rejected or only partially paid. At this time you are responsible for any balance remaining. We strive to provide the highest quality of dental treatment, and in order to guarantee our work we ask that you do your part with regular exams and dental cleanings in intervals recommended by our Dentist and Dental Hygienist.
